I hereby grant permission, without reservation, to the Shorefront YM-YWHA and the United Jewish Appeal-Federation of Jewish Philanthropies of New York Inc. (UJA-Federation) and those authorized by the Shorefront YM-YWHA and UJA-Federation, to take photographs and make recordings of me to use in original or modified forms of all media now or hereafter known, with or without our names or information about us, for the promotion, public education, and/or fundraising activities of both organizations. I understand and agree that I am entitled to receive no compensation for the above.

I release the Shorefront YM-YWHA and UJA-Federation, its officers, directors, agents, employees, independent contractors, licensees and assignees from all claims that I now have or may have in the future, in relation to the above.

I agree that the Shorefront YM-YWHA and UJA-Federation will be the sole owners of all tangible rights to the above mentioned photographs and recordings, with full power of disposition.

Release of Liability: Shorefront Y, its staff, and volunteers are not liable for any injuries, accidents, or medical conditions that may occur during the program. You waive any claims against them arising from your participation.

Informed Consent: By participating in our programs, you acknowledge that you understand the risks associated with physical activities and agree to assume full responsibility for any injuries that may occur during your participation.

By signing below, I acknowledge that I have read and understand the Self-Responsibility and Injury Disclosure for the Israel Day Parade on Sunday, May 18 , 2025, with the Shorefront Y. I agree to assume responsibility for my own safety and well-being of minors that joined me for this event.
